Whooping cough cases increase

If you DO NOT have a Computer Printout - please complete a Request Form, (available from reception)
If you have a covering letter, attach it to the request form.

MAKE SURE YOU HAVE INCLUDED ALL ITEMS WANTED AND WHICH METHOD OF COLLECTION YOU PREFER.

Please allow AT LEAST 2 WORKING DAYS before you collect your prescription either from the Surgery or the Devizes Chemist of your choice.

Requests for repeat prescriptions are NOT taken on the telephone to avoid errors
